How does Betacap TR 40mg Capsule work?

Propranolol works by affecting the body’s response to nerve impulses, particularly in the heart. It slows the heart rate and improves the heart's efficiency in pumping blood, preventing arrhythmia, stabilizing blood pressure, and reducing the risk of heart attacks. It also widens blood vessels, enhancing blood flow to prevent angina and migraines.

For tremors, the exact mechanism is unknown, but it is believed to block nerve impulses to the muscles responsible. Propranolol blocks the effects of adrenaline and noradrenaline, reducing heart rate and anxiety. In treating pheochromocytoma, it helps prevent dangerously high blood pressure by blocking high-adrenaline hormones during tumor removal surgery.

Pharmacology

Propranolol Hydrochloride, the active ingredient in Betacap TR, is a beta-adrenergic receptor blocker widely used in managing hypertension and angina pectoris. It works by antagonizing catecholamines at beta-adrenergic receptors, which helps to reduce the rate and force of heart contractions and decreases oxygen consumption by the heart.

Propranolol is almost completely absorbed from the gastrointestinal tract but is partially metabolized by the liver during its first pass through the portal circulation. Peak plasma concentration occurs 1 to 1.5 hours after oral administration, and the plasma half-life ranges from 3 to 6 hours. The metabolites of propranolol are excreted through urine.

Dosage & Administration of Betacap TR 40mg Capsule

Adults (above 18 years):

• Hypertension: Start with 80 mg twice daily, increasing at weekly intervals to a maintenance dose of 160-320 mg daily.
• Angina pectoris: Start with 40 mg 2-3 times daily, with a maintenance dose of 120-240 mg daily.
• Thyrotoxicosis: 10-40 mg, 3-4 times daily.
• Anxiety: 40 mg once daily, which may be increased to three times daily.
• Prophylaxis of migraine: Start with 40 mg 2-3 times daily, with a maintenance dose of 80-160 mg daily.

Children (from day 1 to 18 years):

• Hypertension:
  • Neonate: 0.25-0.5 mg/kg three times daily, adjusted according to response.
  • Child (1 month to 12 years): 0.25-1 mg/kg three times daily, with a maximum dose of 5 mg/kg in divided doses.
  • Child (12-18 years): Start with 80 mg twice daily, maintaining 160-320 mg daily.

Interaction

Patients receiving verapamil, salbutamol, levosalbutamol, ergot alkaloids, lidocaine, or phenobarbital should be closely monitored during therapy.

Contraindications

• Bronchospasm

Pregnancy & Lactation

Propranolol falls under pregnancy category C and should not be used in pregnant women. It is excreted in human milk; caution is advised when administering propranolol to a nursing mother.
